NJ Labor Audit

 

Provide the last day of the seven day work week and the designated

pay date for paying employees. 

The following information will be required:

1. Time Records - Time cards, time sheets, etc., with daily and

    weekly hours worked by each employee.

2. Payroll Records   - Register/Journal   for each pay period for all

    employees with gross pay, itemized deductions, net pay, and rate of

    pay.

3. Cancelled Payroll Checks for the period:   ______________________

4. Employee Records   - List of all employees for the last two years

    including the employee's name, address, social security number,

    date of birth (if a minor), hire and termination dates, and job title.

5. Federal Identification Number

6. NJ Unemployment  Number  - a copy  of  last  quarterly  filing  of the

    NJ-927  &  WR-30)  and  cancelled check (or proof of electronic 

    payment).

7. Workers.'. Compensation   - (I wll need to see the policy) policy

    number, insurance company name, effective & expiration dates.

8. Bank Information   - Bank names, addresses, and account numbers

    for the payroll account.

9. Company Ownership Information   - Names, titles, home addresses,

    and social security numbers. If LLC,Please include all members’

    names and percentage of ownership

10. Copy of Employee Handbook

11. Working Papers - for all minors past and present.

12 .If your firm contracted any subcontractors, please provide a

     complete list: a copy of all contracts/ agreements (to include

     addresses, phone# and type work performed), workers comp &

     Liability Insurance Policies; Business Registration Certificate:

     Invoices: Hold Harmless Agreement; W-9’s and 1099’s if applicable.
